Not all of the authors who should have been included in the printed volumes of Lexicons came to the attention of the committee(s) in time. The authors whose biographies ought to be included (or their representatives) are asked to submit their answers to the following questions. For your convenience, there are both English and Yiddish versions of this ankete and you are invited to prepare your answers in either Yiddish or English or any combination. Also, you are invited to submit photographs of the author and/or pictures of title pages from some of their work. In all cases “published” refers to both printed material, online web sites, manuscripts, etc. You may save your work and return to it online to complete prior to submitting.

This questionnaire is primarily the same one that was used by the Congress for Jewish Culture since c. 1955, but updated in 2025.

Your answers to the questionnaire may be submitted online, below, or you may write to: lexicon@CongressForJewishCulture.org to request an interview in person, by phone, or by video.

9. Which works and sorts of books (with full title of each book, publisher, year, and number of pages; if the title is not clear, give the nature of each work; counting as works, books translated from other languages, and enumerate foreign language translations of one’s own writings in book form).

10. Playwrights are asked to provide precise information concerning performances of their works.

11. Edited which newspapers, journals, or collections, with tables of contents; when, where, and for how long.

12. If received literary prizes, which ones, when, and where.

13. Community activities – organizations, boards of directors, etc.

14. List of pseudonyms, aliases, and alternate name spellings for author.

15. Bibliography of the most important treatises which others have written about the author or his/her works; reviews, interviews, biographies in biographical dictionaries; encyclopedias, anthologies, and the like. (For each bibliographic entry, provide name of author, title of treatise, where published, publisher, place of publication, date, and page numbers; bibliographical sequence should follow dates of publications.)

16. If necessary, provide other information that is important for the bibliography.

17. Present residence location (City, State, Country). If deceased, where the writer is buried
